01What It Is

What Is IASTM?

If you have stretched, foam rolled, or rested and the same tight, stuck spot keeps coming back, there is likely tissue underneath that will not release on its own no matter how much you work at it yourself. Instrument-Assisted Soft Tissue Mobilization, or IASTM, is the general category of techniques that specific branded systems like Graston, Astym, and Hawk Grip all fall under, and it exists because hands alone can only find and treat so much. Instead of working with fingers and thumbs, your therapist uses a specialized handheld instrument that glides across your skin and picks up on catches, ridges, and rough patches beneath the surface, scar tissue, adhesions, and fibrotic soft-tissue restriction that limit your motion or are quietly driving your pain.

Once a restriction is located, the instrument lets your therapist apply targeted pressure directly to it in a way fingers cannot replicate. That pressure breaks up scar tissue and adhesions, increases blood flow to the area, and stimulates your body's own tissue remodeling and healing response, essentially prompting the tissue to rebuild itself in better condition. It is used across sports injuries, post-surgical scarring, chronic tendon issues, and general muscle tightness that has not responded to stretching or rest alone. Sessions themselves are brief and are always paired with therapeutic exercise in the same visit, since the instrument work opens the tissue up and movement is what makes the change actually last.

02Who It's For

Who it's for.

IASTM is not a first resort, it is for tissue that is genuinely stuck and has not responded to the usual approach. If stretching, rolling, and rest have not moved the needle, one of these three situations is usually why.

01

Old scar tissue limiting motion

Surgery, a bad sprain, or an old injury left behind scar tissue that has tethered itself to the tissue around it, and months or years later it is still quietly limiting how far a joint or muscle can move. IASTM finds those adhesions layer by layer and breaks them up so the surrounding tissue can glide and move freely again.

02

An overuse injury that will not quiet down

Years of repetitive strain from work, a sport, or just daily use can leave fibrotic tissue built up around a tendon, the kind of buildup behind stubborn cases of tennis elbow, golfer's elbow, or Achilles tendinitis that has not responded to rest. IASTM targets that fibrotic tissue directly instead of asking you to simply wait it out.

03

General tightness stretching has not fixed

You have foam rolled, stretched, and rested, and the same tight spot in your calf, shoulder, or back keeps returning like clockwork. That usually means the restriction sits deeper than stretching alone can reach, and IASTM is built specifically to find and address it at that level.

03What To Expect

What to expect in a session.

IASTM is brief and direct, and it is never done on its own. It is one part of a full session built around your specific goals. Here is how a typical treatment runs from start to finish.

01

Evaluation and assessment

Your therapist examines the area by hand and with the instrument itself, since the tool's edge picks up on catches, adhesions, and rough or fibrotic tissue that can be hard to feel by touch alone. This assessment tells us exactly where to focus before any instrument work begins, so the treatment is targeted rather than generic.

02

Instrument treatment

Your therapist glides the instrument across the treated area, applying focused pressure to the specific spots identified in the evaluation. It is a firm, deliberate technique aimed at breaking up restriction and increasing local blood flow, and it is typically only a few minutes spent on the tissue that actually needs it, not a long, drawn-out treatment over the whole muscle.

03

Exercise and home plan

We move straight into stretching and therapeutic exercise while the tissue is warmed and mobile, since that combination of instrument work plus movement is what makes the treatment actually hold. You will also leave with specific exercises to do between visits so the gains keep building instead of fading by your next appointment.

Good to know

Temporary redness or light bruising-like marks over the treated area are a normal, expected part of IASTM, not a sign that anything went wrong. They mean the treatment reached the tissue it needed to, and they typically fade within a few days.
